Once you have been approved by a QMP Utah Green Team Doctor you will need to submit your online application to the Utah Department of Health's Electronic Verification System (EVS)
Utah Green Team Doctors will provide a recommendation within the EVS which will enable you to purchase medical cannabis throughout the state
Upon completion of your Utah medical marijuana card consultation, you must complete the following four simple steps online with the Utah Department of Health:
All patients must first create a Utah ID account before starting an application in the electronic verification system (EVS). A Utah ID account can be created by going to id.utah.gov

Before starting an online application, a patient should review the EVS Patient Application User Guide. After reviewing the guide, the patient may go online in the EVS and begin the medical cannabis card application process.
Log-in to the EVS using your UTAH ID credentials and submit your application. Once your application has been submitted, your QMP Utah Green Team Doctor will complete the physician section.
Once the state fee is paid, your card is made available for use at any medical cannabis pharmacy in Utah! We will notify you when your card is ready for renewal, which takes place annually.

On November 6, 2018, 53% of Utah voters approved Proposal 2 — the Medical Cannabis Act — representing a major victory for medical marijuana advocates who fought for years to establish a compassionate program. Some revisions have been enacted since then.
In 2019, Gov. Gary Herbert (R) signed SB 161 to make a number of modifications to the state's medical cannabis program, such as prohibiting discrimination against medical cannabis patients in family court matters. On February 28, 2020, Gov. Herbert (R) signed SB 121, which makes more changes, including:
In 2023, Utah's legislature updated regulations which will eliminate 6-month card renewal for new medical cannabis patients and work to decrease overall program costs. Instead of requiring patients to complete three 6-month follow-up appointments with their Qualified Medical Provider (QMP) in order to be eligible for a one-year recommendation, the law passed in May of 2023 will allow one-year recommendations to be provided to both new and renewing patients based on the discretion of their provider. This amendment is huge for new or prospective patients who feel medical cannabis is inaccessible due to the high enrollment expenses. Extending medical cards to one year will save both time and money as patients use cannabis to help with their individual health conditions.
Utah's medical marijuana program is still in it's infancy and will continue to mature over time. One big advantage to the program is that patients do not need medical records or documentation to become approved, unless their only qualifying medical condition is PTSD.
